It is also used in the prevention of perforation of the vagina.
The structure of conjugated equine estrogens (CEE) is similar to that of natural estrogens. The conjugate system consists of estrogen conjugates that act in the same way but in a different manner.

It is also to be used in menopause only when the risk of menopausal symptoms is less than 10%. It is used in the prevention of premenstrual dysphoric disorder (PPD). It is used in the prevention of perforation of the lower back and lower-muscular veins when the risk of menopausal symptoms is greater than 10%. It is used in the prevention of perforation of the vagina.
CEE, a conjugated equine estrogen, acts on the low- estrogen receptors in the vagina and other tissues (uterus) and partially on estrogen-receptor complex of skeletal muscle (spongy lining of vagina) by binding to the estrogen receptor subtype-specific. This structure leads to an increase in the binding of [3H] estrone on the hypothalamus and pituitary gonadotropin-releasing hormone (GnRH), which in turn stimulates ovulation and increases the growth of follicles in the ovaries.
The binding of estrone to Premarin on estrogen-receptor complex of skeletal muscle leads to the release of follicle stimulating hormone (FSH) and luteinizing hormone (LH) from the cells and into circulation. Estrogen from Premarin stimulates the growth of follicles in the ovaries and the release of a mature egg from the ovary into the cytoplasm of the penis.
